Photography consent & release form

In consideration of my engagement as a model, upon the terms herewith stated, I hereby give Shannan Petersen and Isla Ann Photography LLC, her heirs, legal representatives and assigns, those for whom the photographer is acting, and those acting with her authority and permission:

a) The unrestricted right and permission to copyright and use, re-use, publish,and republish pictures and video of me or in which I may be included intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in conjunction with my own or a fictitious name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever.*

b) I hereby release and agree to save harmless Isla Ann Photography LLC, his/her heirs, legal representatives or assigns, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.*

c) I grant to the photographer, its representatives and employees full copyright and the right to take photographs of me as deemed fit by the photographer.*

d) Participation is voluntary and I will not receive financial compensation.*

e) Participation in the photography session produced by Isla Ann Photography LLC confers no rights of ownership whatsoever. *

f) I release Isla Ann Photography LLC from liability for any claims by me or any third party in connection with my participation.*

g) I hereby affirm that I am over the age of 18 years of age and have the right to contract in my own name and do so of my own free will.*

h) I understand that re-editing or cropping of images is strictly prohibited. And that I am only to share images as given in their original state. Photographer does not give print release.*

* By checking this box and typing my name below, I am electronically signing the model release form. I understand that an electronic signature has the same legal effect and can be enforced in the same way as a written signature. I have read the above authorization, release and agreement, prior to its execution. I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.*
